Meet Irena: Warm, Evidence Based Care for Adults in Wellesley MA
Hello. I’m Irena Cafasso, a Licensed Mental Health Counselor (LMHC) and the heart behind Irena Cafasso Counseling. I offer high quality, virtual therapy sessions to adults across Massachusetts.
While my base is in Wellesley, MA, my work reaches clients in nearby cities and throughout the state. I hold a Master's degree in Counseling Psychology from Boston College (2002) and have over 20 years of experience helping adults with life's difficulties.
My hope is to offer a safe space where you can be exactly who you are, without judgment, as you move toward a life that fits your deepest wishes.

Why I Do This Work
My interest in counseling began with a strong belief in people’s ability to grow and heal. Like many, I have managed my own difficult times; cultural identity struggles, difficult life changes, and the long journey of self discovery. These experiences gave me a deep respect for the strength it takes to simply face each day, especially when you feel weighed down by worry, hurt from the past, or self criticism.
I've learned that most people aren't seeking a quick cure, but rather a sense of inner peace and the ability to manage life’s upsets. Therapy, for me, is not about fixing a broken person; it's an act of self care. It is setting aside time to gain self awareness and practice self compassion. This careful work, this turning toward yourself with kindness, helps build a healthier life, stronger relationships, and a lasting feeling of purpose.
My history of practice taught me that feeling truly heard and seen creates change. The therapy room, even the virtual one, must be a safe, judgment free zone where your pain and your hope can exist together. My purpose is to help you blend insight into why you struggle with practical, evidence based skills for how to manage those struggles. Whether you are dealing with anxiety, issues around food and body image, or the loneliness of trauma, my goal is to help you live a full life. Safety and pacing are essential; we move at a speed that always feels secure and right for you.
I enjoy working with individuals on cultural identity and acculturation. I speak both English and Czech, and I know the many layers of managing different cultures and the unique difficulties that self esteem and relationship patterns have. I believe in this work’s power to help you find the strength and authenticity already inside you.
Training & Specializations
My clinical background comes from my education and two decades of focused work. This long term commitment to continuing education means the help you get is current and grounded in good practice.
My main training areas and expertise are:

Primary Specialties
Eating Disorders & Body Image
I have worked extensively helping adults improve their relationship with food and their body. My work is inclusive and follows the principles of HAES (Health At Every Size), focusing on well being, intuitive eating, and self acceptance, not diet goals.
Anxiety & Depression
I use concrete, skills based approaches, like ACT and DBT, to help you manage difficult feelings, stop overthinking, and avoid actions that keep you stuck.
Trauma and PTSD
I work from a trauma informed stance, knowing that trauma is not just a memory, but a state that affects your body. Safety is the priority; we work at your speed, using grounding and emotion regulation tools to process the past without feeling overwhelmed, helping you feel control and safety in your body now.

My Approach in the Room
My therapy approach is integrated, compassionate, and collaborative. It does more than listen; it gives you tools to make real, measurable progress every day.
Psychodynamic/Relational Therapy
This is the core of our work. We look at how your past experiences and early attachment patterns affect your current feelings and relationships. Knowing the "why" helps us change the "how."
DBT Informed Skills
I include strong behavioral skills for intense emotions, including distress tolerance and emotion regulation techniques.
These are practical tools you can use right away to reduce chaos and manage difficult moments without making things worse.
Acceptance and Commitment Therapy (ACT)
ACT helps you clarify your core values, work and build mental flexibility.
Here, we learn to handle difficult thoughts and feelings well, giving you energy to take committed action toward a good life, even when distress is present.
Mindfulness & Self Compassion
We practice being aware of the present moment without judgment, pairing this with kindness toward yourself.
This calms the inner critic and builds true inner peace.
My Approach in the Room
In our virtual sessions, available to you across Massachusetts, you can expect:
A Warm, Safe Container
I provide a non judgmental, approachable space where your honest feelings are respected.
Active Collaboration
We are partners. We set clear, personal goals together and check in often to keep the work helpful for your life.
Safety and Pacing
Your comfort is important. You decide the speed of our work, especially when we talk about hard or painful subjects.
Skill Building and Homework
Therapy involves practice. You will leave sessions with practical behavioral skills to use. This active practice outside the session leads to lasting change.
Gentle Challenge
I offer kind, direct feedback to show you things you might not see and gently invite you to try new ways of thinking and relating.
My promise is to give each client my full attention, believing that being truly heard and seen creates power. The final result is a healthier life based on self awareness and self compassion, not one controlled by fear or past pain.
